Self-pay visits and superbills
I do not accept insurance or The Lactation Network. I provide superbills that you can submit to your insurance company to request reimbursement. Coverage depends on your plan, so please check directly with your insurer.
Payment
A credit card is required when booking. Visit fees are charged after your appointment. I provide a superbill for you to submit to your insurer if you wish to request reimbursement.
Cancellation Policy
Life with little ones can be chaotic. If you need to reschedule, please contact me and we will work it out. Cancelling within 24 hours of appointment will incur a charge of 50% of the full booking.
Service Area
Service area includes: San Jose, Santa Clara, Los Gatos, Saratoga, Cupertino, Campbell
I generally travel within 15 minutes of my home in Los Gatos. Any requests for service outside of this area will have additional travel fees. Please contact me for details before booking outside of this area.
